As a CHHA at Better Together you will provide personal care and homemaking services essential to the client’s health care and comfort at home, under the direction, instruction, and supervision of the Registered Nurse.

CHHA Responsibilities Include but are not limited to:

  • Follow the plan of care to provide safe and competent care to the client
  • Help the client to maintain good personal hygiene and assist in maintaining a healthy and safe environment
  • Plans and prepares nutritious meals for the client
  • Provide attention to client’s non-medical needs, including companionship and social engagement
  • Assist client with light housekeeping tasks and medication reminders
  • Assist the client with ambulation, transferring or positioning as approved and supervised by the RN
  • Assists the therapy personnel as needed with the rehabilitative process
  • Encourages the client to become as independent as possible according to the nursing care plan
  • Attempt to promote the client’s mental alertness through involvement in activities of interest
  • Establish communication and a professional relationship with clients, family members, and co-workers
  • Immediately report any changes in client’s mental or physical condition or in the home situation to the RN or Administrator
  • Completes visit notes, daily activity logs, log in and log outs in scheduling software in a timely manner
  • Provide reliable care by being punctual and consistently covering shifts
  • Attends in-services as required by regulation

Certified Home Health Aid Job Requirements:

  • NJ CHHHA License – Current and must be in good standing
  • PPD within last 12 months
  • Proof of Rubella and Rubeola vaccinations as well as other Immunizations
  • Valid driver’s license, car insurance and reliable automobile
  • Be genuinely concerned about helping people and have high moral standards of honesty
  • High school diploma or equivalent
  • Pass an extensive Criminal Background Investigation
  • CPR / First Aid certification
  • Two verifiable work references
  • Professional appearance and professional attitude
  • You must be reliable, punctual, and able to read, write and speak English as needed for the job

As a Companion at Better Together you will:

  • Provide emotional support as clients heal or cope with functional limitations, aging, sickness, or surgery
  • Take care of basic light housekeeping while ensuring a clean home environment for the client
  • Remind clients to take medication
  • Plan and prepare meals while taking into consideration special diets and preferences
  • Help clients engage in cognitive building activities, social events and hobbies
  • Perform tasks to help clients feel safe and supported
  • Have strong communication and planning skills and be willing to meet the physical demands of the position and comply with all safety, medical, and dietary directions
  • Drive clients to planned outings, events, or doctor’s appointments
  • Handle errands for or with the client such as grocery shopping
  • Immediately report any changes in client’s mental or physical condition or in the home situation to the RN or Administrator

Companion Requirements:

  • High School Diploma or equivalent.
  • Fluency in the client’s language may be required.
  • Excellent references and a valid driver’s license with a clean driving record.
  • Strong communication, listening, planning, housekeeping, and reasoning skills.
  • First aid/ CPR certification
  • Pass an extensive Criminal Background Investigation
  • Two verifiable work references
  • Willingness to follow directions and adhere to all company policies, safety procedures, and dietary, exercise, and medication plans.
