How we work

We understand that sometimes we cannot always plan for care before an emergency occurs. This is why Better Together will make every effort to meet a potential client within a 48-hour timeframe. After our initial and in-depth telephone questionnaire, our administrator will visit the place of residence and provide a face to face meeting to learn of personal preferences, routines, living conditions and your loved one’s daily needs. A free home safety assessment will also be completed at that time to ensure the safety of all in the home. Next, a registered nurse will conduct a comprehensive in-home physical evaluation We will gather all this information and work with you/your loved one to provide a plan of care which meets everyone’s expectations and needs. We take the relationship between caregiver and client very serious and will do all we can to make the perfect fit between your loved one and our caregiver.

Once care begins, we will remain in close contact with you and your family. Our Nurse or Administrator will reach out within 30 days of the start of care and make sure you are satisfied with the services being delivered. Our RN will make both scheduled and non-scheduled supervisory visits throughout care and if there are any changes in your loved one’s needs. There will also be care conferences if any care plan changes are necessary.
